I figured it out finally. I’m using iThemes and they have a place to customize CSS. When I looked up their style.css while examining the page using the Google Inspect tool, the code you gave me did not appear, even though I knew it was there. I added the code to the Inspect tool, and the form appeared per your image.

Interesting. I was able to get the desired result when I edited the inspector style sheet.
I tried the style.css for both the plug-in and the theme to no avail.
