How can I add multiple send to addresses at checkout for my customers that purchase the same item for multiple recipients, such as:

Send to:
Bob Smith
xxxx Easy St
North Hills, CA 93111

Betty Sue

South Park, NY 10012

Up to 5 different addresses if possible or more?
