Thanks jrf. I did further research and made necessary changes to my site. Although, the link you provided is a new/better reference. Thanks!

The duplicate titles and descriptions were because the same description and title appeared on all paginated content. I fixed this by modifying templates to add page number at the end of title and description for all pages except the first.

Thanks for for getting back. I disabled the “Hide Backend” option in Better WP Security.

RobertKn is right. W3 Total Cache could cause this problem. Take a look at this post:
