Hi! I just tried the form on another page and I got the same result. Also tried uninstalling the plugin and installing it again. No error messages, just a blank square with borders.
Could this have something to do with my hosting (GoDaddy)? Or some weird CSS?
Solved it by using URL instead of TEXT input!
