It is very unlikely that your issue has anything to do with EWWW IO, but from what you describe (and show in your screenshot), it seems like a javascript error. Please open the inspector in Chrome or Firefox (Ctrl+Shift+I), and then click on the Console. Once you have that open, refresh the media library, and see what shows up (either on page load, or when you try to click the upload tab/button).
It generates error:
“Uncaught TypeError: Cannot read property ‘getRelatedTab’ of undefined.and
“Uncaught TypeError: Cannot read property ‘hasClass’of undefined.
