I am having the same problem. I had to disable all my plugins to figure out what the problem was. I finally figured out it was this one and then narrowed it down to the login page renamed. When I tried to go to toe /wp-manage for my website I got a epic-404 error, and couldnt get to /wp-admin either when i turned off the login page renaming I could get back to wp-admin. I have been using the renaming for several months now and havent had problems until yesterday
I am now having the same problem, we just starting taking donations on Monday and the first ones worked fine. The most recent one was not recorded, I just got the paypal email. I have been running on 3.0.0 since it was released. I will send you my log file as well.
Did you ever launch the donor wall? What is the shortcode?
