The format for the date isn’t the same format as when the input is in the billing section and both the date and time inputs can’t be selected.

This is a client’s active site, so I don’t want to leave the calendar in order notes for an extended period of time when it is broken.

WP, WooCommerce and your plugin are up to date.
I believe it would be block checkout. The Divi theme builder was used to create the page.
