I think I may have sorted it but now I want to have field hearder in bold and would like a space after the last input field before preferred format.

Thanks for that mc_nate. I did manage to do a work around by also changing some of the wordpress templates.
One small question though is that the layout of the widget seams to not look great in the sidebar. Eg I have email, f_name L_name Company and then title.
All except title look good. This maybe due to the size of the text Title so the input field is therefore moving to the space.
I tried to change the title input header to Title / position and although its changed it in mailchimp it does not appear to update in the pages I have included the widget.
Any ideas?
