I’m a colleague of @rossgurung and am also looking into this issue. Would you be able to let us know if there’s an alternative way to send you the diagnostics report via the Login Security plugin? The options you’ve described don’t seem to be available in the lighter version of the Wordfence plugin that we’re using.

In the meantime, I’ve done some additional digging and came across this related thread which may be relevant since we’re also hosted on WPEngine.

While we can technically resolve the issue by wiping the WFLS tables in the database, we’ve now experienced this on two different websites. Each time, all admin users have had to reset their MFA codes. I’d prefer not to ask our clients to reset their details again without being confident that the underlying bug has been identified and fixed, and we dont have to risk them having to set up their codes a third time.

I’ve encountered this bug in several of my projects as well. Is there a plan to include this patch in an upcoming update? If so, could you please provide an estimated timeline for the next release?
