This seems to happen because there are no quotes around the text for each field on the CSV. I’ve seen the same issue by creating my csv from an excel and saving as .csv which saves the file as plain text when viewed in VI. The only difference between it and the example is the example has quotes around the text for each field.
I wonder if the intent was more for new user setup where the users would be expected to change their passwords after login. Just going out on a limb here.
