When I deactivate the plugin: lightbox 2, it works properly.

I’ve looked through the documentation to find info on how to switch to the default theme… but all I could find was info about reverting it back to a simple form.. which kinda defeats the purpose of my form 🙂
I did try deactivating all the plugins with no luck 🙁

I did not set this up… I’m just trying to fix it 🙂 so forgive me I’m not up to speed…
The template is currently set to ‘no template selected’
I’ve tried each one with no luck.
To my knowledge it hasn’t been updated lately… but it is up to date.
