after I updated it to version 2.3.9 (and today 2.3.10) it has been working fine.

I just reinstalled 4.0 and it works fine.

Dan,
I updated to version 4.1.2, but still the same error

Fatal error: Class ‘DG_AudioVideoThumber’ not found in /home/evankel2/public_html/wp-content/plugins/document-gallery/inc/thumbers/class-audio-video-thumber.php on line 4
