It looks fine to me if that is any help. I would leave it like that tbh.

It works well without AMP but I found here https://amp-wp.org/ecosystem/themes/ that oceanwp should work with AMP. (Official amp plugin)
I turned off all plugins except AMP and the problem remains.

What I meant was those text boxes under:
Form Builder/Form Settings (Field Type: Dropdown)/Choises
Now, under Text and Value there are fields where we can input choices but that is too short. Could you extend them from 13 visible characters to 40 or more, for convenience sake I struggle editing in that field?
