As long as you are adding the shortcode to the template, it will appear on all posts. If you want it to appear on a specific post, you will need to use the shortcode in the post itself instead of inside the template.

Yes this is possible in the pro version. If you go to Integration > Email, you will see a URL assigned to each choice. You may use that URL to create a button in your profile page. Clicking the button will cast a vote to that specific choice.
