the plugin translations are hardcoded in/admin/js/lang-all.jsyou could possibly add the Czech translation there.
1 year ago, last time, when the plugin was updated by the author, there was no solution to do it in any other way.I’d recommend you to try some different plugin which is actively developed and supports at least the translation using the POedit or Loco Translate.

In language option of the plugin (in loco) just find the Slovak translation and hit copy.
Than in WordPress language choose Czech as normally, but in “Template options” select Copy target translations from “sk_SK”

if you want to translate just a few words not entire plugin,
you might create translation using Poedit. In plugin directory find the .POT file and using Poedit you will create translation to your language name it like this mondula-form-wizard-cs_CZ where cs_CZ is the code of your language. Translate everything you need and then paste .po and .mo files to the same folder as the .POT file.Other method is to use well known plugin Loco Translate and here’s begginer’s guide.
If you’ll translate it on https://translate.ww.wp.xz.cn/projects/wp-plugins/multi-step-form (where are 3 Portuguese mutations available), you’ll need to reach 100% + ask for permission to get PTE rights

After these changes can anyone suggest new translations.If you ask the translators to contribute at translate.ww.wp.xz.cn and the contributors will want to translate the entire plugin, You can make a request to give them PTE(Project Translation Editor) rights to translate (not just make suggestions) your plugin.
When the language reaches 95% translated strings it will automatically generate language pack. On this page you can see all of the generated language packs for your plugin. This is handy thanks probably to WordPress 3.9+ which allows to update/install language packs, same page as plugin/theme updates appear and you as a developed don’t need to include translations in
/name_of_your_plugin/langfolder.Since version 4.6 WordPress prefers community translations instead of
/name_of_your_plugin/langfolder.If you’ll have some problem with generation of translations you should take a look at WordPress Slack’s channel named #meta-language-packs or if you’ll need some more technical info use the #meta-i18n channel.
