Having the same issue here too, but updated to 3.1.4. Header and footer are present in the Free Channel Theme from themejunkie.
Anyplace else to check on this?
I disabled minify on my install and it seems to have solved it on the outset. But obviously, something is wacky.

The home page
The article titled “ART Episode 44 – Burnt Banana Bread (Video)” Has a thumbnail. The article “ART Episode 43 – Amiga Overload! (Video)” below it, doesn’t have a thumbnail, yet it appears when you go to read the article by clicking on it’s title. The same graphic for 2 different articles, but it only appears in one article.

I go into the Media Library from the dashboard menu, click “show” on the image I want to use and then click on the button “Insert Image”.
