yes, when I change it, it works but when I open the ticket in zendesk, these fields are empty.

I have 7 extra fields. Should I custom them again?

The message is: “Successfully connected to Zendesk. You can now add a form to any page by using the shortcode [zendesk_request_form]”

I have 2 forms in the website, one of them works perfectly but the customize form doesn’t work and has the commented error…
