This is working now! Two things I changed:
1. I am using the new version of the plugin (1.0.9)
2. In the plugin settings, ‘Strip shortcode in excerpt’ needs to be set to ‘No’.

Those are the settings that I currently have. The image is actually at the top of the post but it has a caption and the plugin seems to skip over it.
Hey @johnh10, can you post what you changed to get the plugin to work? I am also trying to achieve the same thing but am not very good at PHP.

That did not work unfortunately. It still ignores the first image with the caption.
