I clicked on Quick Edit and found the template dropdown right next to the page order.For others that might have this problem, go to pages to display all of the pages, then hover over contact and click on Quick Edit. In Quick Edit under Template choose contact and click update.

Thank you for replying. I tried to do what you typed and I do not have a page attributes section on the edit/new page page. Would you know if this is a module I need to download and install?You help is greatly appreciated!

In reply to: [evolve] How to activate the Contact pageI am at a complete loss on how to activate this really awesome page. I see it in my customize sidebar but I have no idea how to make it work? Am I missing a checkbox somewhere?

I updated the theme and the disappeared on me as well. To turn them back on go to your dashboard, Appearance > Theme Options > Front Page Content Boxes and enable front page content boxes. Then you have to go through and check box Enable box 1, Enable box 2, Enable box 3, etc.
