Thanks for the suggestion but this is the intended behaviour of the plugin.
The plugin is using WordPress native way to recover the password.
The modal window it’s only intended to by used for the login.

Have you tried to save the permalinks?
This usually fix this kind of WordPress errors. You can do it going to Settings > Reading > clic on “Save Changes”.
