Tried placing the shortcode at different places, but its not showing up. Would be great if you could refer me where it should be added.

Had tried that. But it appears on wp-login page, but not in the modal/popup of SimpleModal Login. SimpleModal login has its own page where it sends the post/get parameters to wp-login.
