Same issue for me: I can successfully submit sites, I get the message “Your URL was sucessfully submitted for editor review!”
I also receive the e-mail with all the information entered, but I don’t see the sites in the admin section, so there’s no way to approve them.

Usually this is inserted in the Footer file (footer.php), just above the closing body tag.
If you only want to count visits, to your main page, put it in index.php.

Maybe you could try the WP-dTree plugin? I haven’t tried it myself, but it looks like something you could use.
If that doesn’t help, you might want to look into tagging (instead of categories)? There are a few plugins to organize them.
