I eventually resolved this issue. I believe the first time I tried to activate it something went wrong, and the only way I could delete it was manually on the server.
I finally tried re-installing the plugin and then, before activating, I deleted it via the admin plugins interface, which must have run some cleanup code.
After that, I was able to install fresh and activate it without issue.
