Got it!
I had all my headers in place, but the checkbox was PART of a section with another element (so in this case it doesn’t send the value in the email).
So now I created a separate SECTION and I placed ONLY the checkbox element in it and it worked (it properly sends the value in the email).
I think it would be useful if this is added in the documentation for more clarity.
