Mine did end up being theme related. I am using a commercial theme and there is other functionality the sidebar does that the custom sidebars plugin was conflicting with. Thanks for your replies!

In the WP Dashboard, click the “Gallery” then in the links it drops down, click “Manage Gallery”. You will be brought to a page that lists your galleries. Click one to edit it. In the top section of the screen titled “Gallery Settings” and you will see a description box you can type in. Make sure you are using the template=extended in your shortcode so the description will display.
