“Self-hosted” is the same as having your very own Piwik server.

Ad. b) yes and no, WooCommerce Piwik plugin allow to use of js/index.php, but you have to inject this path at your own. Eg. if you use Wp-Piwik and set it to serve tracking code with
/js/index.phptracker code this plugin will still do its work. But there is no setting inside this plugin that allows to select path to tracker. By deafault this plugin serves standard tracking code.@Soft79 Did you try to disable this option in Integration setting page?
Add tracking code to your site. You don't need to enable this if using a 3rd party analytics plugin (i.e. Piwiktracking plugin)Forum: Plugins
