I tested both live and sandbox.
I couldn’t quite deactivate all plugins. I think the two left were: ‘Advanced Custom Fields PRO’ and ‘Secondary HTML Content’. When these were deactivated, the form failed to show at all.
No, I didn’t try switching to TwentyTwentyOne. I’ll do a back up of my site and try that, thank you.

Everything Searchamn said, is still true 7 months on. I’ve had major issues with this theme, and replies to tickets take 4 to 6 DAYS, and the answers are so glib that they clearly don’t read or understand the content of the issue. They just see a key word, then cut and paste an answer somehow related to the key word – not the issue.
Avoid this theme. Avoid StyleMix themes.
