Updates:
Redownloaded the old version wordpress-seo.1.4.14 and change the latest and fixed the issue.
Hope to fix this soon.
BTW, I forgot to mention that I’m running with the latest version (wordpress-seo.1.4.15)

I imported all of my post “All Content” and published it on a different “dummy” site to test it out and the results is:
– If no WordPress SEO the generation of the url is fine before and after publish
After activation WordPress SEO (default settings)
– the problem occurs again even added with different or mix up settings.
