Yes i agree with catacaustic. I faced a similar issue and culprit was a third party plugin which i was using on my site to upload images.

I think you should focus more on improving your product rather than doubting your plugin users.
Nevermind with issues, i have uninstalled the plugin because i don’t think it is worth my effort.
