@dpruth001: did it worked for you with the bulk aktion “regenerate thumbnails”?
for me, the plugin workes perfectly for fresh uploaded PDF, but for all existing pdfs it does not generate the thumbnail-image.
I get this error on the regenerate page (wp-admin/tools.php?page=regenerate-thumbnails):
Changes failed: {ATTACHMENT_ID} is not a valid image-id
