In reply to: [MW WP Form] 多言語プラグイン poly lang と使用すると言語設定が無視されるこの症状はpolylangのバグ（っぽい）事がわかりました。
ショートコードを用いると、polylangの多言語機能が（おそらく）殺されてデフォルト言語にリセットされる為、上記のような現象が発生しているようです。
（WP-Membersなどのショートコードでも再現）よって、今更ですがresolvedさせて下さい。お騒がせ致しました。

WordPress 3.5.8 polylang 1.5.1 MW WP Form 1.7.0その他もろもろ、W3 total cacheやMo cacheなども入ってますが、停止しても再現。
なお、プレビューだとうまくいく（英語になる）のですが、公開すると日本語に戻ってしまいます。
