Ok just do the following things
1)deactivate ithemes plugin
2)You said that your are using Maxcdn so temporarily just point your DNS to your bluehost Ns’s not maxcdn.In nutshell just deactivate Maxcdn for your site
I thought this may worksAlso untick the following settings you showed in the above image
enable hackrepair.com blacklist Banhost Ban UsersFirst of all the plugin will only work not notify the servers or anything.It just record the blacklisted ip in .htaccess and in database.
You can try to remove Maxcdn for a while and coz the server will not directly serve your content instead passes through the cdn.So just try to remove it and tell me the resultForum: Plugins

You said your ip was blacklisted but none of the above images were showing that your ip is blacklisted.
If your ip is blacklisted due to ithemes security you can clear using the following method
just go to phpmyadmin and select your database
then select the following table “xxxxxx_BWPS_lockouts” where xxxxxx is your table prefix
then Delete the record with your IP address from the xxxxxx_BWPS_lockouts table
