We have tried to put a ticket in with the link that has been provided. There seems to be an issue with the customers code so it is not allowing us to submit a ticket.
What would be the best way to proceed?
There is still an issue with the activity log as I have explained above.

The issue I am talking about is when you expand in the activity log it has an icon and it seems to not be loading and formatting and displaying this part fully.

The issue regarding the icon seems to be a problem with version 5.4.0 onward hence not being mentioned from the start.

I have tested the updated version v5.4.1, which has not fixed this issue. It is still not returning any of the notes. There is still an issue with getting the icons to display as well.

We have followed your advice to try and see if this was caused by other plugins on the site.
After further investigation of the issue, we found that the issue is happening during WooSync while it importing data from WooCommerce to the Jetpack CRM it is re-importing the notes that the customer would have added which is causing it to show up as repeating.
We found this as by pausing the WooSync was no longer repeating the notes but once reactivated every time it was syncing data it would add the note again.
This seems to be an issue when it is updated to 5.2.0 as when the CRM is on version 5.1.0 we are not having this issue. We found this by doing this on a clowned site to test this issue to not impact the live production site.
