Your report shows PayPal is active, but Subscriptions Mode is disabled. Could you confirm the plugin is fully up to date? Sometimes intermittent field mapping issues are resolved in recent releases. – It is update to date.
The conflation of first/last names into the first name field suggests a possible conflict with how PayPal is returning customer data. Do you have any customizations to checkout fields, or other plugins (like form builders or address validators) that might be intercepting checkout data? – I do not have any customized checkout fields.
Your report shows PayPal Vault is disabled and Subscriptions Mode is off. Are you running in live mode now, or still testing? If test mode was active during some of the affected orders, that could explain inconsistent data capture. – It is live. I’m not sure how PayPal Vault or Subscriptions Mode would affect this. could you provide more info?
Since you have multiple payment gateways enabled (Stripe, WooPayments, PayPal), it’s worth temporarily disabling the non‑PayPal gateways and retesting a PayPal order to see if the issue persists. Do the same for all other plugins and see. – I’d prefer not to do this as customers use those payment gateways. Is there a way to test this without disabling the other gateways?
I noticed logging is disabled for PayPal in your report. Enabling logging: https://share.zight.com/z8u2ZRe5 could give us clearer insight into what data is being returned from PayPal during checkout. – I turned it on. I’ll post the log as soon as I get an order.

Also, it appears this issue (the missing phone number) started when PayPal changed their settings menu at the end of October.

Thanks for the quick response!

I went through several payments and it seems like it only happens on PayPal. However, while almost all PayPal transactions, there are a couple which were not affected. Regular credit or debit card transactions seem like they are processed correctly.

It looks like the first transactions which were missing phone numbers were about Oct 22. The transactions which were conflating the first and last names into the first name field were at least Sep 29, perhaps earlier. I don’t know if those dates coincided with plugin updates (my plugins auto-update).
