I also tried many things didn’t work.
In the end I found that the name of the flag image wasn’t perfectly correct.
I used “de_LS.PNG” in it didn’t work.
Then I tried “de_LS.png” and it worked.
So obviously we need to take care that we use small letters as image file extensions.
Apart from this I did everything how it was written within the polylang description.
Maybe this hint helps you as well.

Sounds weird. Only German isn’t displayed while other languages are displayed? What version of Contact Form 7 do you use?
