Thanks to your help, I was able to find the culprit plugin, WP Security.
Deep within the settings, it had an option to manually approve all new user registrations site wide.
I was able to disable this function and now your plugin seems to be operating properly.

Not using a Cache plugin at the moment…
I cannot find what could be causing this…
The Affiliate says Active (see screenshot)

This plugin is unusable in this state.
How do we fix this?

I don’t think you understood.
It IS and WAS set to Active
. AND in the backend it says that the Affiliate is “Active” BUT the system still treats the Affiliate as Pending. So unfortunately your answer doesn’t help because the steps you advised were already performed.
This issue is still not resolved.
