I’m gonna run the latest version of this plugin again later today. I’ll see if it works for me this time. Fingers crossed!
I did see a demo of it prior to the update and Benjamin Bradley (from iThemes.com/webdesign.com) mentioned adding meta info to get those extra lines added. If the straightforward set up of the form doesn’t work as expected, then I’ll see if the meta data part of it will work…

Thanks so much for working on a fix. I’ll give a it a go and let you know how it works for me. I have another site with a similar need for custom fields in the checkout of Exchange.
