Also, having done the above, I’m no longer getting an error on the page, instead the content area is just blank. So an improvement :).

Thank you so much for helping me through this. I’m still not getting the form to show up so wanted to check with you if I have done this correctly.
My permalink for my ad is http://firstmedtransport.com/join-our-team/455. So the shortcode is [aol_form id=455].
My page content on my actual Join Our Team page looks like this:

It is public and published as is the Ad. Its permalink is http://firstmedtransport.com/join-our-team and its slug is join-our-team
Do I maybe not have the slug correct?

I did what you mentioned but still no luck. I think this might be the problem:
My page is http://firstmedtransport.com/join-our-team but the actual ad is forcing a URL of http://firstmedtransport.com/join-our-team/join-our-team because that is what I’ve called the Ad. And if I leave off the Ad name, the ID of the Ad is being added to the permalink.
I’ve got the short code for the form on my actual Page but I’m thinking it doesn’t know what to render because the page is one link and the Ad is another.. ? Thoughts?
