同じコトで悩んでテストしてみたのですが、
・フォームに項目を追加してみた→CSVには反映されず
・さらに管理者宛の通知内に、同じ項目を追記してみた→CSVに項目追加された
という結果となりました。
取り急ぎお知らせまで～。
