You’re right, I didn’t get the attachments into my site through the default WP media upload.
I always prefer uploading via fileZilla – because it’s closer to how I’m used to copy/move documents on my laptop, and because fileZilla never asks any questions regarding file sizes etc.

I had the same problem with missing PDF thumbnails on my site (the problem occured approx. December 2015), and none of the above solved my issue – although I never got to check out which Ghostscript version is installed on the server.
Anyway, I accidentally stumbled upon the (or a) solution, which is that the PDF file names must be written without spaces. As soon as replaced spaces with f.ex. hyphens, a thumbnail was successfully generated for each and every PDF file.
