I have the same problem.
Plugin version 2.8.1 and WP 4.0
Before i have update to 4.0 everything was fine. And e-mail was sent. In form details was: Emailed To: [email protected], right now there isn’t any email address just: Emailed To:
I found the problem. It is that my hoster or wordpress itself doesn’t allow to connect to db with login whiсh is different than wordpress db login.
