I’m assuming by your reply that this is valid evidence of a compromised site 🙁

Does anyone know which methods could be used to instigate a hack like the one I’ve experienced?

Could it be the result of local malware on a workstation that was used to access the admin of the site? Or would it more typically be the result of a compromised FTP/cPanel/SQL password? Does anyone recognize this particular hack? I haven’t been able to find anything in my searching that matches it.
