The issue is now fixed. It was a mod_security rule on the server causing such weird behavior. My host has fixed the issue for us.

Nothing has changed in the last 2 days. We are using the latest version of the plugin.
The log simply says:
Navigated to http://www.domain.com/wp-login.php
