Yes, this is still happening.

Every time someone makes a booking/reservation, we get an email saying the following (even though nothing was cancelled).

Subject: Appointment Status Changed
The status of the following appointment has changed.
New Status: Cancelled

Any advice on how to stop this?

I was using Firefox version 41.0.2. I just upgraded to Firefox 44.0.2 and that fixed the layout.

Figured out the solutions to both!

For the booking form extra fields, I wasn’t selecting YES for “show on registration form” drop down.

Thanks for the quick reply! My client is very VERY happy with this appointment plugin.
