But as per your comments, it looks like I have to create another page to submit the job with shortcode [submit_job_form].

Actually, it was working before and it stopped working when I disabled the plugin and change the theme. I will try to troubleshoot again and see.

Facing issue since mid August 2015. Can Author of this plugin fix it.

Now every blog is working fine and I am able to login without any issue.

It temporary resolved after commenting lines in wp-login.php for one blog.
But now its working fine for all blogs without any issue or change. Not able to understand what was wrong.

Once I activated it decreased the mobile page speed and as well as Mobile user experience.
Any suggestion ?
