I also have this duplicate order problem and i only have the free version not premium. So it would be really nice to actually get a solution here rather than assume its premium versions installed as most of your responses say.
Upon updating your plugin, duplicate orders, orders on hold etc started happening with my existing customers and new customers. If this cant be resolved i will just go find another plugin. So far looks like terrible support.
