Well, a couple hours after I reset all settings, the little icon is not shown.
However, I can still see the icon in the source code; it’s just not shown on the page:

In reply to: [LiteSpeed Cache] Guest Mode adds an icon to the pageEdit: It looks like Guest Mode is not the culprit. I turned it off and the little icon would show up in a while anyway. The only way to get rid of it is emptying entire cache or deactivating the whole plugin.

It seems that another plugin called “WP Optimize” has caused the caching issue. After I disabled it the problem was gone. I’m looking at alternatives to it. Many thanks!

@dshanske yes, this is my theme. I modified it a lot but didn’t do much about the comment/discussion part.
