thank you very much for your help and guidance. I can confirm that the issue with the 500 error is now gone, and the forms are working correctly.
I also tried adding the suggested code to the .htaccess, but unfortunately it immediately brought back the 500 error, so I removed it again. For now I will leave the file empty since everything is working fine this way.
Hopefully, this will prevent the problem from coming back, but for now everything looks good. Thank you again for your support and patience.

thank you for your reply. I would like to confirm what I did step by step:
- I went to the
/wp-content/uploads/forminator/directory.- There were two subfolders (
230_67bf8e0322bccace5223df6fd90c4233and319_2677adbf08063ffe3915ab7ce1ccbf60) and one.htaccessfile directly in theforminatorfolder (but not in the subfolders).
- There were two subfolders (
- I downloaded this
.htaccessfile from theforminatorfolder.- I uploaded it to Google Drive, here is the link: https://drive.google.com/file/d/1vbU5e95I-8BOKiyAujdpwwu-jcOVBHpU/view?usp=sharing.
- After downloading it, I opened the
.htaccessfile on the server, removed all the code inside, and saved it as an empty file (so now it’s still.htaccess, but with no content).
Could you please check the file I shared and confirm if this is the correct file and the right procedure?

I would like to follow up on my previous ticket.
- The SSL certificate is now fully installed and working (the site loads securely over HTTPS, and external mixed content checkers show no issues).
- I have removed all caching plugins and also added cache-control rules in
.htaccessto make sure it’s not a cache issue. - I completely uninstalled and reinstalled Forminator.
However, the problem still persists:
- For every form, a 500 error is thrown when trying to load the CSS file in
/wp-content/uploads/forminator/[formID_xxxxx]/css/style-[formID].css. - For example, my test form with ID 230 gives:
https://tj-trener.cz/wp-content/uploads/forminator/230_xxxxx/css/style-230.css 500 (Internal Server Error) - The same happens with a new default form created from the template, so it’s not related to customizations.
The forms themselves can be submitted, but the CSS file always fails to load with a 500 error.
Could you please advise what might be causing the CSS file generation/loading to fail?
It looks like Forminator is unable to generate or serve those CSS files correctly on my hosting environment.Thank you in advance.

I have used the code and, to be honest, I’m not a professional developer, but I have checked and it appears the
Options -ExecCGIline is no longer present. So it seems to be working correctly.I have now sent the error logs to the email address you provided.

It turns out the form still doesn’t work correctly. When I first activated the Hummingbird plugin, the form on my site appeared all red, as I had set most colors to red in the Forminator settings to easily see if my changes were taking effect. However, when I reverted the colors to their original state, the form broke again, reverting to a style where the dropdown menu and radio buttons are completely unusable.
I then tried clearing all caches, and the form’s appearance changed to another style (which I did not set), where the radio buttons and dropdown menu were usable. But, when I tried to make any changes to the form itself (like deleting test fields), it reverted back to the broken, unusable state.
I’ve exhausted all my troubleshooting options and I’m losing hope of getting the forms to work. I believe there might be a deeper conflict between plugins or my theme.
Would it be possible to provide you with temporary admin access to my site so you can investigate the issue directly? This seems to be the only remaining option to find a solution.

It finally works! The problem was that I had only installed and activated the Hummingbird plugin. I had to click on it and go through the setup process, and after that, the Forminator settings finally started to change the form’s appearance on my site.
I am not sure if the code you sent was part of the final solution, but I’m going to leave it in place so I don’t break anything again.
