Ok based on your reply I found the problem.
I have installed the plugin “Title Remover” which helps me hide the title of an post/page. When the title is hidden (allthough it is set) there is no title on the thumbnails. If I set title to visible and then go to settings-> related posts -> save then the thumbnail has title.
And finally I managed to solve the problem. Unistalled “Title Remover”. I managed to hide titles with css. So everything is ok now.
