I ran into this too, it seems that some paths have changed and forms created with older versions stopped working because of that (I saw some 404 “file not found” errors in the Apache error log).
I was able to fix it by uninstalling and reinstalling the plugin and creating new forms (note that it did not work with the default contact form after reinstalling, I had to delete that and create a fresh one.)
